How to Join Lifevine Collective Network

As of June, 2010 medical authorizations must be printed on special tamper-proof paper. This precaution greatly reduces the risk of falsification; therefore, we no longer have to verify patients' identity. That speeds up and simplifies the enrollment process greatly.

If you are a patient interested in joining Lifevine Collective Network, you may initiate the process in several ways:

1. The preferred way

Please email scanned documentation to info@medmj-wa.com or fax to (206) 299-3990. We need to see the following:

  1. a copy of your doctor's authorization;
  2. a copy of your WA State Driver's License or State ID;
  3. your current contact information;
  4. current contact information for your doctor.

Once we receive the above paperwork, we will contact you to make an intake appointment with us. You must come in with the original documentation, printed on tamper-resistant paper. This allows us to confirm the authenticity of your authorization, qualifying medical condition and identity. Upon joining the Network, Lifevine members sign a membership agreement, which explains their legal rights and responsibilities under WA State law, RCW 69.51a. Lifevine's permanent ID card is optional and costs $10.

2. The acceptable way

Because of the added security of tamper-proof authorizations, Lifevine now accepts walk-ins. Our office is located on Capitol Hill, 109 15th Ave E., across the street from Group Health. Our hours of operation are Monday-Friday, 11-6, Saturday 12-5. Make sure to bring all of the above original documentation during office hours, and we can complete your enrollment process right there and then.

3. The longer, but tried-and-proven way

Of course, you may still choose to mail us copies of your documentation and request an appointment. It will undoubtedly take a bit longer, because we collect mail from our post office box once a week, but is guaranteed to get your paperwork to us. We will contact you for an appointment, as soon as we receive it, as long as you include contact information, such as a phone number or email address, where we can reach you.

4. Still need a medical authorization?

If your condition can be legally treated with medical marijuana under the Washington State law, but you don't have a doctor's authorization, you need to have the discussion with your doctor. If he/she sees fit to recommend medical marijuana, your doctor must fill out an official authorization form. Make sure the current version of the form is being used. The form must be printed on special tamper-resistant paper only your doctor can obtain.

If you don't have a doctor willing to authorize your use of medical marijuana, our information on finding a physician may help you find a more open-minded medical provider. It has become a little easier since I-5798 became law in June of 2010.
